Stay Prepared Ahead of Your First Visit
We want to thank you for considering Dr. Carlson and his team for all of your oral health needs. It is very important to us that you have a smooth and stress-free experience whenever you have an appointment, and we aim to do so from your very first visit! Below, you can find all the information you need as you become a new patient. Your First Visit
When you first arrive, you’ll be welcomed at the front door with a smile from one of our friendly team members. Following the completion of a new patient form, you’ll be brought to an operatory right away. Dr. Carlson will then come into introduce himself and ensure you’re feeling comfortable. After discussing your main dental concerns, an assistant will capture X-rays, then you’ll come back to the chair so Dr. Carlson can perform a detailed exam. If needed, a treatment plan will be created and explained to you.
New Patient Forms
While you’re welcome to complete new patient forms at our dental office, you can expedite your appointment by downloading and filling them out ahead of time! We’ve made this process incredibly easy – simply print them out by clicking the link below. These forms ask questions related to your oral and medical history, any medications you are taking, and main concerns you’d like addressed at our dental office.
Dental Insurance Welcome
Dental insurance is designed to make your dental treatments not only more affordable, but predictable as well, especially when it comes to preventive care. If you make use of a dental insurance plan, there’s a pretty good chance that we either accept or are in-network with it!
